Chat: Endometrial Activation

The following transcript is from an online chat hosted by the American Fertility Association on March 18, 2008.

Session Date: March 18, 2008—Tuesday
Guest Speaker: Gad Lavy, M.D., New England Fertility Institute
Topic : Improving Implantation and IVF Success: Endometrial Activation
[Tue Mar 18 16:55:59 PDT 2008] Corey_Whelan wow! Welcome to all of our early birds! I see Dr. Lavy is here as well. Hi all. I’m Corey Whelan, The AFA’s Director of Development and your chat moderator tonight. I am very pleased to introduce Dr. Gad Lavy of the New England Fertility Institute. We officially start in around five minutes, but please feel free to post an early question. Welcome everyone!
[Tue Mar 18 16:56:33 PDT 2008] Corey_Whelan While we are getting started, I wanted to mention that The AFA’s Family Matters Conference has been booked for Sunday, October 5th at Steiner Studios in New York. Please be sure to save the date!
[Tue Mar 18 16:57:17 PDT 2008] glavy Welcome Everyone Endometrial activation was discovered serendipitously when endometrial biopsies were performed prior to an IVF cycle. The success in cycles following the biopsies were higher. This led to some additional research that has confirmed the original findings. The exact mechanism of endometrial activation is not entirely clear.
[Tue Mar 18 16:57:55 PDT 2008] Corey_Whelan Dr. Lavy, can you explain what endometrial activation is exactly?
[Tue Mar 18 16:58:47 PDT 2008] glavy Creating a small area of injury activates certain proteins in the endometrium that are apparently essential for implantation
[Tue Mar 18 16:58:56 PDT 2008] Patty joined.
[Tue Mar 18 16:59:10 PDT 2008] mwelch Dr. Lavy is endometrial activation currently being utilized in fertility programs in the United States, as the research was originally done in Israel.
[Tue Mar 18 16:59:16 PDT 2008] Corey_Whelan Hi Patty. We are just getting started.
[Tue Mar 18 16:59:47 PDT 2008] glavy The original study was done to evaluate the endometirium of women whose IVF cycles were not sucesssful
[Tue Mar 18 17:00:24 PDT 2008] glavy The original studies were performed in Israel at the Weitzman Institute of Science. The team was led by Dr. Nava Dekel.
[Tue Mar 18 17:01:05 PDT 2008] glavy Following the original biopsy the women had undergone another IVF cycle and most of them conceived.
[Tue Mar 18 17:01:24 PDT 2008] pennyevans joined.
[Tue Mar 18 17:01:45 PDT 2008] sb10847 joined.
[Tue Mar 18 17:01:49 PDT 2008] mwelch If a patient elects this procedures, at what point is it done in the IVF cycle. . In other words how long between biopsy and transfer.
[Tue Mar 18 17:01:51 PDT 2008] rackle joined.
[Tue Mar 18 17:01:51 PDT 2008] Corey_Whelan Hi Penny, welcome to our chat with Dr. Gad Lavy. To see the transcript so far, just click on the action button. Ditto sb.
[Tue Mar 18 17:02:07 PDT 2008] Corey_Whelan Hi Rackie
[Tue Mar 18 17:02:55 PDT 2008] toni joined.
[Tue Mar 18 17:03:22 PDT 2008] Corey_Whelan Hi Toni, welcome. Everyone please feel free to post your questions.
[Tue Mar 18 17:03:30 PDT 2008] glavy The biopsy is generally done in the luteal phase of the cycle preceding the IVF cycle. We have experimented with doing the biopsies several days prior to the transfer and have also observed excellent results in poor prognosis patients
[Tue Mar 18 17:04:37 PDT 2008] Corey_Whelan To[Private] mwelch Hi Marion. Some of the others are starting to post questions, why don’t you hold back for a few minutes.
[Tue Mar 18 17:05:24 PDT 2008] sb10847 Is is just a strait forward biopsy?—what does this “activate” per se?- I am fascinated by the autoimmune/cytokine issues with the endometrium and transplantation—I have had a biopsy done in conjunction with co-culture
[Tue Mar 18 17:05:55 PDT 2008] mwelch To[Private] Corey_Whelan Just getting ready when it gets slow. I am so excited about this new technology.
[Tue Mar 18 17:06:19 PDT 2008] pennyevans I had a retrieval on Saturday that retrieved 12 eggs and 4 were mature and 8 were immature. The 4 that were mature they used icsi and only one fertilized. My RE called yesterday to say that it divided further and if it makes it to day 5 they will transfer it. I have 3 children and 8 losses and am 43.5 years old. She said if I have the 1 to transfer she will then start progesterone. Is there anything else at this point that I can do to promote implantation if I hopefully have an embryo on day 5?
[Tue Mar 18 17:06:30 PDT 2008] glavy Hi sb. As I said it is not clear exactly what is being activated. It appears that certain genes responsible for induction endometrial proteins are being activated
[Tue Mar 18 17:07:10 PDT 2008] Corey_Whelan To[Private] mwelch cool! keep that enthusiasm, it makes for a great chat! I’m really glad you are here to help move things along and get the right information out, that’s terrific
[Tue Mar 18 17:08:22 PDT 2008] glavy Hi Pennyevans It appears that the problem was with egg quality probably related to your age. Endo activation may help but would probably not be effective in such cases.
[Tue Mar 18 17:09:13 PDT 2008] tenayagirl left.
[Tue Mar 18 17:09:14 PDT 2008] rackle What types of situations would endo activation be effective?
[Tue Mar 18 17:10:17 PDT 2008] glavy Hi rackle as all new procedures we have concentrated on poor prognosis patients.
[Tue Mar 18 17:10:59 PDT 2008] glavy These are patients who have had repeated IVF failures. It makes sense that those patients with “good” embryos would be the best candidates
[Tue Mar 18 17:11:31 PDT 2008] pennyevans Is there anything you would recommend to me at the age of 43.5 to help with implantation during an IVF cycle. I spontaneously conceived and miscarried 2 months ago and thought IVF would help with a good embryo and if I get that embryo on day 5 what may help with implantation which would be the next challenge of the ivf cycle. I try to remain positive since I had implantation naturally 2 months ago.
[Tue Mar 18 17:13:10 PDT 2008] sb10847 With your poor prognosis patients on whom this bx is done—do you prefer to do day 3 transfers?
[Tue Mar 18 17:13:23 PDT 2008] Enter_Name_here joined.
[Tue Mar 18 17:13:40 PDT 2008] shaymer01 joined.
[Tue Mar 18 17:13:44 PDT 2008] glavy You should be positive. Spontaneous pregnancy is definitely an excellent predictor of success. I feel that endo activation is worth trying as it has no downside and can only help
[Tue Mar 18 17:14:27 PDT 2008] Corey_Whelan Hi ENH and shaymer!
[Tue Mar 18 17:14:49 PDT 2008] shaymer01 hi corey!
[Tue Mar 18 17:14:55 PDT 2008] Corey_Whelan nice to see you.
[Tue Mar 18 17:15:07 PDT 2008] shaymer01 you too!
[Tue Mar 18 17:15:21 PDT 2008] kaylalunaava joined.
[Tue Mar 18 17:15:39 PDT 2008] Corey_Whelan Hi kay, welcome back!
[Tue Mar 18 17:15:51 PDT 2008] glavy The original study was done in 12 women and was followed by a larger study of 45 women and 89 controls. The results show that endometrial activation was associated with a significant increase in success. Women in the study group were twice as likely to conceive as the controls.
[Tue Mar 18 17:15:56 PDT 2008] kaylalunaava Thanks
[Tue Mar 18 17:16:32 PDT 2008] pennyevans At my stage in the ivf cycle of this Thursday being day 5 if I still have an embryo are there any recommendations for this cycle to promote implantation?
[Tue Mar 18 17:16:43 PDT 2008] sb10847 Who is the lead author on this study?
[Tue Mar 18 17:17:10 PDT 2008] CP-NJ left.
[Tue Mar 18 17:17:19 PDT 2008] mwelch Dr. Nava Dekel, as mentioned .
[Tue Mar 18 17:17:25 PDT 2008] glavy Nava Dekel. The original article was published in Fert and Steril in 2003
[Tue Mar 18 17:17:35 PDT 2008] sb10847 thanks
[Tue Mar 18 17:18:13 PDT 2008] glavy welcome
[Tue Mar 18 17:18:19 PDT 2008] glavy good luck
[Tue Mar 18 17:18:55 PDT 2008] shaymer01 not sure if I missed it, can endo activation be explained? what is it?
[Tue Mar 18 17:19:39 PDT 2008] Corey_Whelan guys, always feel free to click on the action button and scroll through the transcript of the chat if you missed anything. And don’t be afraid to ask any question!!!
[Tue Mar 18 17:21:00 PDT 2008] sb10847 Two question—1. top three factors affecting endometrial receptivity and 2. do you gravitate towards higher dose or lower dose IM progesterone support s/p IVF?
[Tue Mar 18 17:21:14 PDT 2008] toni left.
[Tue Mar 18 17:21:37 PDT 2008] MC Hi Dr. Lavy- is this procedure particularly helpful for those with PCOS undergoing IVF? Thanks.
[Tue Mar 18 17:22:19 PDT 2008] glavy Progesterone supplementation has to be tailored to the levels of estrogen in the cycle. For high estrogen cycles, a higher dose of progesterone is best.
[Tue Mar 18 17:22:47 PDT 2008] glavy Endometrial receptivity is affected by estrogen progesterone and blood flow delivering these hormones to the endometrium
[Tue Mar 18 17:23:22 PDT 2008] mwelch To[Private] Patty hi patty, Do we know each other ?
[Tue Mar 18 17:23:30 PDT 2008] rackle left.
[Tue Mar 18 17:23:34 PDT 2008] Patty To[Private] mwelch yes
[Tue Mar 18 17:23:39 PDT 2008] pennyevans left.
[Tue Mar 18 17:23:39 PDT 2008] glavy I think that the PCOS patients deserve special attention. Their PCOS should be controlled prior to starting IVF. In PCOS there are egg factors as well as endometrial factors

[Tue Mar 18 17:24:27 PDT 2008] rini28 hi there
[Tue Mar 18 17:24:34 PDT 2008] Patty To[Private] mwelch I agree he has talked about it during the tpr meetings
[Tue Mar 18 17:24:35 PDT 2008] rini28 this is just chat correct?
[Tue Mar 18 17:24:38 PDT 2008] glavy To[Private] mwelch hi marion. What do you think??
[Tue Mar 18 17:24:46 PDT 2008] rini28 no dial in?
[Tue Mar 18 17:24:56 PDT 2008] Corey_Whelan yes, that is right.
[Tue Mar 18 17:25:21 PDT 2008] mwelch To[Private] glavy Great. I am very pleased. Corey wants me to hold my questions since it got busy . I wil wait till the end .This will ge posted on their site tomorrow..
[Tue Mar 18 17:25:22 PDT 2008] rini28 so what is the steps
[Tue Mar 18 17:25:31 PDT 2008] rini28 for improving our chances
[Tue Mar 18 17:25:35 PDT 2008] mwelch To[Private] Patty There will be kit pick up tomorrow for the patient tonight. Dr. Browns patient. She will call before she comes. I am bushed….
[Tue Mar 18 17:25:55 PDT 2008] glavy To[Private] mwelch thannks for being here
[Tue Mar 18 17:26:12 PDT 2008] rini28 my 1st son was an IVF baby and now trying for 2nd
[Tue Mar 18 17:26:13 PDT 2008] pennyevans I think my question was lost amongst others. If my embryo makes it to day 5 which is Thursday, do you have any suggestions to promote implantation after the transfer other than progesterone support which she said she would start Friday since I don’t know if my embryo will be viable until day 5.
[Tue Mar 18 17:26:16 PDT 2008] Patty To[Private] mwelch you home now or at the office
[Tue Mar 18 17:26:30 PDT 2008] CP-NJ left.
[Tue Mar 18 17:27:01 PDT 2008] mwelch To[Private] Corey_Whelan Corey what is the dial in??t
[Tue Mar 18 17:27:32 PDT 2008] Corey_Whelan To[Private] mwelch she was wondering if this was a telephone support group. They do phone.
[Tue Mar 18 17:27:34 PDT 2008] glavy Sorry Pennyevans. sounds as if you are getting the right advice. I generally use a small dose of estrogen in addition to the progesterone to improve endometrial receptivity
[Tue Mar 18 17:28:07 PDT 2008] mwelch To[Private] Corey_Whelan We would love to of help following the chat if appropriate.
[Tue Mar 18 17:28:18 PDT 2008] Corey_Whelan To[Private] mwelch not sure what you mean?
[Tue Mar 18 17:29:25 PDT 2008] rini28 what about olive oil as opposed to sesame seed oil in the progesterone?
[Tue Mar 18 17:29:26 PDT 2008] pennyevans What type and dose of estrogen do you use and what days of the cycle. I will mention it to my RE if I have a day 5 embryo. Thank you.
[Tue Mar 18 17:29:53 PDT 2008] glavy i use estrace 0.5 mg twice daily
[Tue Mar 18 17:31:07 PDT 2008] pennyevans Is the estrace 0.5 an oral medication and what days of the cycle do you recommend taking it?
[Tue Mar 18 17:31:08 PDT 2008] rini28 any holistic advice on improving implementation?
[Tue Mar 18 17:31:14 PDT 2008] JoanK With a failed IVF cycle, in some cases, is it possible to actually say implantation failure was a “cause” of why it was not successful?
[Tue Mar 18 17:32:00 PDT 2008] glavy Estrace is given orally. I use it from the very beginning of the cycle.
[Tue Mar 18 17:32:15 PDT 2008] Corey_Whelan I think Rini was next, then Joan, Dr. Lavy.
[Tue Mar 18 17:33:18 PDT 2008] rini28 tx
[Tue Mar 18 17:33:19 PDT 2008] glavy JoanK : implantation failure may be a result of embryo factor or endometrial factor. In most cases the fault is with the embryos. However an endometrial component should be suspected in cases where embryo quality appears to be good
[Tue Mar 18 17:33:58 PDT 2008] glavy Rini: the type of oil does not matter. Some women tolerate olive oil better that sesame or peanut oil
[Tue Mar 18 17:34:10 PDT 2008] mwelch To[Private] Patty home. id id get alot done today. We are moving through.
[Tue Mar 18 17:34:48 PDT 2008] rini28 tx
[Tue Mar 18 17:34:52 PDT 2008] glavy Acupuncture may help implantation by improving blood flow to the uterus
[Tue Mar 18 17:35:06 PDT 2008] shaymer01 does diet effect the health of the endometrium?
[Tue Mar 18 17:35:11 PDT 2008] Patty To[Private] mwelch we certainly are. by the end of the week we should be in great shape
[Tue Mar 18 17:35:16 PDT 2008] pennyevans Do you use estrace from the beginning to the end of the cycle? Would it be beneficial for me to start estrace at this point near a transfer?
[Tue Mar 18 17:35:53 PDT 2008] mwelch To[Private] glavy I always need to learn. Dr. Lavy mention our help line at the end Helpline@nefertility.com or info @nefertility.com.
[Tue Mar 18 17:36:30 PDT 2008] glavy Pennyevans: you need to speak to your doctor about that. It really depends on your estrogen levels. I am a firm believer in the beneficial effect of estrogen in priming the ovaries as well as the endometrium
[Tue Mar 18 17:36:45 PDT 2008] kaylalunaava left.
[Tue Mar 18 17:36:45 PDT 2008] kaylalunaava joined.
[Tue Mar 18 17:37:32 PDT 2008] kaylalunaava Is Progesterone in oil given IM absorbed differently than Endometrin?
[Tue Mar 18 17:37:34 PDT 2008] glavy The effect of diet is not well researched. I am not aware of any studies looking at this connection. My personal impression is that diet low in refined carbohydrates is beneficial for IVF success in general
[Tue Mar 18 17:37:39 PDT 2008] mwelch To[Private] Patty I agree. I will be in Shelton office in am on pcos meeting. then back to get the checks done.
[Tue Mar 18 17:38:00 PDT 2008] Patty To[Private] mwelch ok
[Tue Mar 18 17:38:29 PDT 2008] sb10847 What is your opinion about physical activity and endometrial receptivity post transfer?
[Tue Mar 18 17:38:33 PDT 2008] woodigit joined.
[Tue Mar 18 17:38:44 PDT 2008] Corey_Whelan Hi woodigit. Welcome back.
[Tue Mar 18 17:39:08 PDT 2008] glavy Kaylalunaava: Both preparations contains natural progesterone. Their absorption is different as the progesterone in oil is released gradually and provides a constant level in the blood. The endometria is absorbed in spurts. Whether that has an impact on success is not clear.
[Tue Mar 18 17:39:24 PDT 2008] rini28 left.
[Tue Mar 18 17:40:05 PDT 2008] kaylalunaava Thanks Dr. Lavy. The PIO then is more effective, correct?
[Tue Mar 18 17:41:27 PDT 2008] glavy I think so. It appears that the studies show that vaginal progesterone is probably close and oral progesterone is not nearly as effective
[Tue Mar 18 17:43:41 PDT 2008] fjp joined.
[Tue Mar 18 17:44:01 PDT 2008] JoanK I have had multiple failed IUI’s plus 2 failed IVF cycles. I am “unexplained” diagnosis. One cycle I even did PGD and they had 3 “great” embryos, and it still did not take. any thoughts?
[Tue Mar 18 17:44:20 PDT 2008] Corey_Whelan Hi fjp. We have about fifteen minutes left with Dr. Lavy so please feel free to ask a question at any time. You can read the past transcript of the chat by pressing the action key.

[Tue Mar 18 17:45:15 PDT 2008] glavy JoanK: You are a PERFECT candidate for endometrial activation

[Tue Mar 18 17:46:07 PDT 2008] JoanK Dr– what is endometrial activation???
[Tue Mar 18 17:46:13 PDT 2008] mindy327 Hello I’m joining late in the session. Just going to read along for now.
[Tue Mar 18 17:46:19 PDT 2008] sb10847 Re-post: Opinion about endometrial receptivity and level of physical activity post transfer—how do you counsel your patients?
[Tue Mar 18 17:46:20 PDT 2008] glavy I Shaymer: I am not in favor of LARGE doses. The dose needs to be titrated to your blood levels. I don’t think that a combination of POI and vaginal progesterone is necessary
[Tue Mar 18 17:46:48 PDT 2008] pennyevans The estrace that you use is it from the beginning to the end of the cycle?
[Tue Mar 18 17:47:08 PDT 2008] kaylalunaava Are there any tests to determine whether or not the uterine environment is conducive to implantation?
[Tue Mar 18 17:47:15 PDT 2008] glavy JoanK: Please check the transcript for the details. EA is an endometrial biopsy done prior the IVF cycle. It has been shown to imrpove implantation in patiens with prior failed cycles

[Tue Mar 18 17:58:36 PDT 2008] frances1 I have three frozen embryos conceived with my own eggs and husband’s sperm at age 45 and have a gestational carrier. We will be having these implanted into her soon depending on the thaw results. Do I have a better chance with a young uterus? I had a hysterectomy.
[Tue Mar 18 17:59:24 PDT 2008] lisamarie I’ve heard the age of the uterus doesn’t matter.
[Tue Mar 18 17:59:30 PDT 2008] Corey_Whelan Dr. Lavy, thanks again so much for being here tonight, what an interesting chat! Again, you are all welcome to stay as long as you like. Please log off with a state roll call, if you don’t mind—I’d love to know where you are all from!! And also, please do consider making a small donation to the afa for bringing this weekly series to all of y’all.
[Tue Mar 18 17:59:36 PDT 2008] Corey_Whelan thanks so much guys.
[Tue Mar 18 17:59:44 PDT 2008] glavy Gestational carrier are picked primarily based on their obstetrical history. Their age is only secondary. Age does not appear to impact the chances of success with GCs
