First Appointment Welcome Packet for New Patients

(And Returning Patients, too!)

Welcome to NEFI! In order to facilitate the registration process and save you time on the day of your appointment, we ask that you complete and return the forms listed below at least two weeks prior your appointment.

Please note that we have included a form which will allow you to obtain your medical records from your physician. Having those records available in advance is invaluable and will allow us to better serve you.

• Female Patient History
• Male Patient History
• Medical Record Release

• A copy of the front & back of your insurance card
